അപേക്ഷകൾ വെള്ളക്കടലാസിൽ തയ്യാറാക്കി അറ്റസ്റ്റ് ചെയ്ത മാർക്ക് ഷീറ്റ് (2 കോപ്പി – 1 ശാഖക്ക്) സഹിതം അപേക്ഷകർ താമസിക്കുന്ന സ്ഥലത്തെ ശാഖാ മുഖാന്തിരം അയക്കേണ്ടതാണ്.

കേരളത്തിന് പുറത്ത് ശാഖകൾ പ്രവർത്തിക്കാത്ത സ്ഥലങ്ങൾ നിന്നുള്ളവർ അപേക്ഷകൾ നേരിട്ട് PE&WS സെക്രട്ടറിക്ക് അയക്കാവുന്നതാണ്. കേരളത്തിനകത്ത് ശാഖകൾ പ്രവർത്തിക്കാത്ത സ്ഥലങ്ങളിലുള്ളവർക്ക് തൊട്ടടുത്ത ശാഖ മുഖാന്തിരവും സമർപ്പിക്കാവുന്നതാണ്.

2021-22 വർഷത്തിൽ സ്‌കൂൾ, സബ് ജില്ലാ, ജില്ല, സംസ്ഥാന യുവജനോത്സവങ്ങൾ, സ്പോർട്സ് & ഗെയിംസ്, എൻ സി സി, സ്‌കൗട്ട് തുടങ്ങിയുള്ള എക്സ്ട്രാ കരിക്കുലർ ആക്ടിവിറ്റീസിൽ ലഭിച്ച സർട്ടിഫിക്കറ്റുകളുടെ അറ്റസ്റ്റ് ചെയ്ത കോപ്പിയും എല്ലാ അപേക്ഷകരും അപേക്ഷയോടൊപ്പം സമർപ്പിക്കേണ്ടതാണ്.

മാർക്കില്ലാത്ത ഗ്രേഡ് സമ്പ്രദായമായതിനാൽ ഒരേ ഗ്രേഡുള്ള ഒന്നിലധികം അപേക്ഷകർ വന്നാൽ മേൽപ്പറഞ്ഞ സർട്ടിഫിക്കറ്റുകൾ കൂടി പരിഗണിച്ചായിരിക്കും അവാർഡ് നിർണ്ണയം.

മേൽപ്പറഞ്ഞ രീതിയിൽ വിദ്യാഭ്യാസ അവാർഡുകൾക്കുള്ള അപേക്ഷകൾ ശാഖാ മുഖാന്തിരം നൽകി ശാഖയുടെ ശുപാർശ സഹിതം സീൽ വെച്ച് വേണം PE & WS സെക്രട്ടറിക്ക് അയക്കേണ്ടത്.

വിദ്യാഭ്യാസ സഹായങ്ങൾക്കുള്ള അപേക്ഷകൾ വെള്ളക്കടലാസിലെഴുതി മാർക്ക് ഷീറ്റ് അടക്കം ശാഖയിൽ കൊടുത്ത്, ശാഖാ കമ്മിറ്റി ചർച്ച ചെയ്ത് അപേക്ഷകരുടെ സാമ്പത്തിക സ്ഥിതി, സമാജം അംഗങ്ങളാണോ എന്നിവ പരിശോധിച്ച ശേഷം പ്രസിഡണ്ട്/ സെക്രട്ടറി/ ട്രഷറർ എന്നിവരിൽ രണ്ടു പേരുടെയെങ്കിലും ഒപ്പും ശാഖാ സീലും സഹിതം PE & WS സെക്രട്ടറിക്ക് അയക്കേണ്ടതാണ്.

ശാഖാ ഭാരവാഹികൾ മേൽപ്പറഞ്ഞ അപേക്ഷകൾ പരിഗണിക്കുമ്പോൾ അവാർഡുകൾക്കും സ്കോളര്ഷിപ്പുകൾക്കുമുള്ള നിബന്ധനകൾ(അപേക്ഷകന്റെ ഭവനത്തിലെ പ്രായപൂർത്തിയായ എല്ലാവരും സമാജം, എഡ്യൂക്കേഷണൽ സൊസൈറ്റി എന്നിവയിൽ അംഗങ്ങൾ ആയിരിക്കണം, ഭവനത്തിൽ തുളസീദളം വരുത്തണം തുടങ്ങിയവ) പൂർണ്ണമായും പാലിച്ചിട്ടുണ്ടെന്ന് ഉറപ്പ് വരുത്തേണ്ടതാണ്.
